Default 1998 high-mileage civic has startup noise

I have a great, tough, 1998 Civic LX that has over $250K miles. Just
this week it has started making an awful racket on start-up, that goes
away once the engine has been running for a couple of minutes. I have
noticed that it is using up oil more quickly, and that it has less
power than it had 100K miles ago (go figure, right?).

Any ideas?

Its loud enough that you hear it while you're driving away from your
house? Have you ever changed your PCV, and how full is your oil on the
dipstick?
