1990 Ford Taurus SHO TIMING BELT WALKING OFF TOP

1990 taurus sho timing belt walked off the top pulley (but only half way) slicing through the timing belt covers. Not sure why this happened. Occurred soon after new install of timing belt. Also installed new tensioner when timing belt installed. Anyone seen this issue?
